Programmes and services
Most of our programmes and services are delivered by your general practice – this is your first point of call for all health concerns.
To support the work of our general practice network Pinnacle MHN also directly delivers some services in the community. Your GP or practice nurse will refer you to any services you are eligible for or may benefit from. Our extended care teams work alongside general practices, focussing on better integration within communities to support long-term condition management plans and child health. Teams include health professionals such as clinical pharmacists, peer support workers, community health workers, dietitians, exercise consultants and more.Support to screening

In the Waikato we offer screening support services for BreastScreen Aotearoa and the National Cervical Screening Programme. We provide targeted follow up, outreach and support to priority group women who experience barriers to accessing breast screening, cervical screening, colposcopy, and assessment and treatment services.Mobile immunisation service
In the Waikato we have a team who help to achieve immunisation targets by following up families/whānau who have children that have not accessed health services for immunisations, or who have missed immunisations. The team also actively work to reconnect families/whānau engagement with their general practice to make sure no other health checks or services are missed.School-based health services
